Your responsibilities will include: • Perform comprehensive physical, social, psychological, cognitive, and developmental assessments as appropriate. • Assess and evaluate wound-related patient needs and overall health status. • Gather information from patients, families, physicians, and other members of the interdisciplinary team. • Develop individualized wound-care plans using clinical judgment, critical thinking, and evidence-based practice. • Coordinate patient care with physicians and other healthcare professionals. • Provide and/or delegate care consistent with the established plan of care and professional scope of practice. • Implement appropriate wound-management interventions and evaluate patient response. • Monitor clinical outcomes and patient/family satisfaction. • Modify care plans as needed based on patient progress and changing clinical needs. • Educate patients and families regarding wound management, treatment plans, and ongoing care needs. • Coordinate referrals and follow-up care when additional support is required. • Effectively manage time, supplies, equipment, and other resources. • Maintain accurate and timely clinical documentation. • Promote patient safety, quality care, and professional nursing standards. • Collaborate with and help coordinate the interdisciplinary care team. • Practice in accordance with legal, ethical, regulatory, and professional standards.
