Responsibilities:
Provide direct patient care and nursing services across various healthcare homes and client locations
Administer medications, treatments, and procedures in accordance with physician orders and care plans
Monitor and document patient vital signs, symptoms, and progress in medical records
Collaborate with interdisciplinary healthcare teams to ensure continuity of care
Educate patients and families on health management, medications, and post-care instructions
Adapt quickly to new clinical environments, policies, and patient populations
Maintain compliance with state, federal, and facility-specific healthcare regulations
Requirements:
Active Licensed Practical Nurse (LPN) license in good standing in the state of Georgia
Previous clinical experience in a hospital, long-term care, or home health setting preferred
Willingness and ability to travel to multiple homes or patient locations as required
Strong clinical assessment, documentation, and patient communication skills
BLS/CPR certification required.
Reliable, self-motivated, and able to work independently with minimal supervision
Flexibility to work on-call as needed
