Health and Wellbeing CoachThe Health and Wellbeing Coach provides personalised support to individuals to empower them to take control of their own health and wellbeing with a focus on lifestyle behaviour change, inclusive but not limited to the core areas of:Healthy Eating and NutritionPhysical ActivityWeight ManagementResponsibilitiesAs a Health and Wellbeing Coach, you will facilitate people in establishing and attaining health‑promoting goals set by the person to change lifestyle‑related behaviours to reduce health risks, improve self‑management of chronic conditions, and improve quality of life. You will deliver face‑to‑face, telephonic, and digital health coaching support to individuals from a wide range of community and healthcare settings, via a combination of one‑to‑one and group‑based support in line with need, location and offer.To provide a health coaching service for eligible participants, you will support people to gain and use the knowledge, skills, and confidence to become active participants in their health and wellbeing so that they can reach their self‑identified health and wellbeing goals, including, but not limited to:Triage and assessmentLifestyle behaviour change support (e.g., stop smoking, move more, achieve a healthy weight/diet, reduce alcohol consumption levels)Problem solvingMotivational interviewingSignpostingDelivery multi‑channel, personalised health coaching support to individuals and groups in line with their needs and preferences, including:Face‑to‑face, telephonic, and digital health coachingMulti‑mix communication methods including in‑app messaging, text, and emailShare helpful and supportive resources in line with goal, stage of journey, confidence, and motivationAdvocate the use of behaviour tracking tools (online or offline)Appraise individuals and population caseloads daily to ensure participants are provided with the right intensity and frequency of support to drive sustained behaviour changeA strength‑based approach that maximises opportunities to utilise local community assets and the wider health and wellbeing economyTo deliver services in line with quality standards, national and government guidelines, and role boundaries, with the ability to identify and act when coaching is not appropriate and to sensitively discuss alternatives or escalations, inclusive of onward referral to broader services.Support with outreach community engagement work, including, but not limited to:Screening services (e.g., NHS Health Checks)Community and stakeholder eventsExternal meetings and networksRoadshows and pop‑upsBusiness and Contract PerformanceWork in line with business and contract performance requirements, including:Implementing Standard Operating Procedures (SOP)Fulfilling policies and quality management framework expectations (e.g., data protection, information security, health and safety, safeguarding)Participating in regular audits and performance observations, demonstrating a proactive commitment to providing high‑quality services and continuous professional developmentAdopting a proactive approach to supervision and training with a demonstrated commitment to personal development, achievement of quality standards, high performance in line with agreed objectives and Key Performance Indicators (KPIs)Reporting and Data CollectionTo ensure accurate reporting and data collection in line with contractual and case management system requirements, ensure:Caseloads can be managed efficiently, effectively, and at scaleTeam members can support population behaviour change as required and in line with needWider informal and formal care givers/specialists can complement and enhance the behaviour change journeyService users/participants only have to tell their story once;
