Principal Function: This role requires a unique set of skills and personality traits to assess, prioritize, and stabilize patients' needs quickly and accurately by providing appropriate health care and instructions to patients. Must possess comprehensive knowledge of medical procedures, as well as an incredible level of compassion and empathy. Will need to display a friendly, open demeaner and the willingness to tackle difficult decisions and urgent situations.
Essential Duties and Responsibilities: Duties include, but are not limited to:
1. Assess patients' medical conditions vis phone or in-person consultations.
2. Prioritize patient treatment based on the severity of symptoms and medical history.
3. Documents patient interactions and maintain accurate medical records.
4. Provide medical advice and guidance within scope of LPN or RN practice.
5. Coordinate with healthcare providers to ensure timely patient referrals.
6. Perform basic nursing duties such as wound care, administering medications, and monitoring vital signs.
7. Educate patients on self-care practices and preventative health measures.
8. Manage patient follow-ups and provide continuity of care.
9. Assist in maintain a clean and safe healthcare environment.
10. Collaborate with other healthcare professionals to ensure comprehensive patient care.
11. Other duties as assigned.
Supervisory Responsibilities: This job may have supervisory responsibilities.
Education and /or Experience: For RN Associate of Science in Nursing (ASN) or Associate Degree of Nursing (ADN) or equivalent from a two-year college or technical school; or one-year related experience and /or training: or equivalent combination of education and experience is required. Previous experience in a health-related environment is desired. For LPN 12-18 months completion of state approved practical Nursing program.
Current Indiana license for Licensed Practical Nurse (LPN) or Registered Nurse (RN)
Salary negotiable.
