Description

Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who see it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Unit Supervisor of Health Home Care Management to join our team located in Queens, NY.

Health Home Unit Supervisor is responsible for the supervision of care management staff and the daily care management operations of the program. This includes direct supervision of care management staff, ensuring quality of care management services provided to clients, management of program compliance requirements, and staff training. The Unit Supervisor will ensure the provision of adequate support to complex clients and clients in crisis.

Essential Duties and Responsibilities

  • Ensure coverage and provide care coordination for assigned care manager caseloads and/or cover caseloads as needed
  • Provide crisis intervention to clients in psychological distress; review and approve client documentation as assigned.
  • Review assessments, plans, progress notes and other documentation as required to ensure necessary detail, content to meet regulatory requirements and standards
  • Develop and Implement quality assurance and quality improvement activities
  • Consult with multidisciplinary team on client's care plan/needs/goals. This may include participation in case conferencing. May also include family members and social supports.
  • Provide supervision and support to staff assigned, including but not limited to assisting in hiring, scheduling, evaluating, ongoing staff development, team meetings and individual supervisory meetings

Education

  • Bachelor''s Degree required
  • Master''s Degree preferred

Job Type: Full-Time

Salary Range: $66,300 - $80,186.55

Sign-On Bonus: $1,500 Sign-On Bonus for Eligible External Applicants - Terms and Conditions Apply.
