The Unite Us Program Coordinator is responsible for the day-to-day management and growth of the Unite Us coordinated care network for Social Determinants of Health (SDOH) across Palm Beach County. This role serves as the primary point of contact between the Health Care District, community-based organizations, and the Unite Us vendor team, ensuring the platform is fully utilized, properly maintained, and continuously expanded to meet the evolving needs of residents and partner agencies.
The Coordinator drives partner enrollment, monitors platform adoption and data quality, manages the active resource directory, and works closely with the vendor to troubleshoot issues, roll out new features, and onboard new services. Success in this role is measured by network growth, consistent and accurate platform usage among partners, and the overall health of the closed-loop referral system.
In addition, the position provides day-to-day coordination and administrative support for the District''s Sponsored Programs initiative, including funding application administration, contract and reporting support, partner communication, payment coordination, data tracking, committee support, and maintenance of reporting systems. The position assists with program monitoring, community partner engagement, and implementation of initiatives designed to improve community health outcomes and access to services.
