Position: Travel Social Worker
Location: Near Winner, SD
Setting: Long-Term Care (LTC)
Contract Length: 13 Weeks
Schedule: Days | 5x8s
Guaranteed Hours: 40 Hours per Week
Pay: Up to $45/hour (based on experience)
Start Date: ASAP
Complete psychosocial assessments for residents upon admission and as needed.
Develop and implement individualized care plans addressing residents' emotional, social, and psychosocial needs.
Provide counseling, emotional support, and crisis intervention for residents and their families.
Coordinate discharge planning and facilitate transitions of care.
Connect residents and families with community resources and support services.
Advocate for resident rights and promote quality of life.
Collaborate with physicians, nursing staff, therapy teams, and administration to ensure comprehensive resident care.
Maintain accurate documentation in accordance with state and federal regulations.
Assist with care conferences, family meetings, and regulatory compliance initiatives.
Bachelor's Degree in Social Work (BSW) required.
Master's Degree in Social Work (MSW) preferred.
Current South Dakota Social Work license (or eligibility to obtain prior to start) preferred, as required by state regulations.
Previous experience in Long-Term Care, Skilled Nursing, Rehabilitation, or Geriatric Care preferred.
Strong communication, organizational, documentation, and interpersonal skills.
Ability to work independently while collaborating effectively with interdisciplinary teams.
