About the Opportunity:• We are partnering with a leading Long Island health system with an extensive and growing network of hospital-affiliated physician practices, ambulatory care locations, and specialty services.
• The organization provides coordinated healthcare across primary care, specialty medicine, diagnostic services, and hospital-based care throughout Long Island.
• As its ambulatory and population health programs continue to expand, we are seeking an experienced Registered Nurse to join the Value-Based Care / Population Health team.
• This position focuses on Medicare Advantage, preventive care, chronic disease management, Annual Wellness Visits, patient outreach, and closing gaps in care across a large outpatient patient population.
• This is an excellent opportunity for an RN with experience in Primary Care, Ambulatory Care, Population Health, Medicare/Medicare Advantage, Quality, Care Management, Community Health, or Chronic Disease Management who wants to use their clinical knowledge in a Monday–Friday, non-bedside environment.
What Makes This Role Different:• Rather than traditional bedside nursing, you'll focus on keeping patients healthy and connected to the care they need.
• You'll review patient information to identify missing preventive or chronic care needs, conduct patient outreach, support Annual Wellness Visits, coordinate screenings and follow-up care, and work with primary care teams to close important care gaps.
• Your work will directly support improved patient outcomes across a large and growing ambulatory network.
Experience & Qualifications:• Active New York State RN license required
• Prior experience in population health, value-based care, care management, ambulatory care, or chronic disease management strongly preferred
• Experience identifying and addressing gaps in care and coordinating preventive/chronic care services
• Comfortable reviewing patient data and working from reports/worklists to identify patients needing intervention
• Strong patient outreach, education, care coordination, and follow-up skills
• Ability to work independently and navigate multiple resources/systems
• Strong EMR/documentation skills
