Summary:

Pay Rate: $29 to $30 Per Hour

Work Mode: On-site Position

Shift Hours: Full-time - 11 PM-7 AM - every other weekend

Responsibilities:

  • Perform quality assurance and peer reviews, evaluate telehealth and virtual patient observation program, and recommend improvements.
  • Meet with senior leadership regularly to discuss Virtual Hospital Program quality metrics and provide feedback on workflow expansion.
  • Lead case reviews with hospital partners, implement solutions, and monitor outcomes.
  • Triages and assigns cases for virtual monitoring/observation.
  • Collaborate on telehealth policy and workflow development and implementation.
  • Maintain operational database including telehealth program performance and quality metrics.
  • Identify concerning trends and participate in corrective action plan development and implementation.
  • Utilize telehealth platforms and technology to achieve company goals.
  • Complete Virtual Care Coordinator orientation successfully.
  • Provide training on telehealth workflows and platform application to new staff.
  • Serve as a liaison between physicians, clinical staff, Digital Technology Partners, and vendors on telehealth issues.

Requirements:

  • 2 years telehealth experience required.
  • Direct patient care experience.

Preferred Skills:

  • Facilitate camera utilization by analyzing virtual patient care reports and monitoring real-time data.
  • Navigate multiple technology platforms and work with healthcare providers to ensure quality care.
  • Provide “White Glove Service” by offering real-time support to troubleshoot telehealth platform issues.
  • Provide continuous observation and surveillance of patients to ensure safety and prevent falls and injury.
  • Monitor clinical early warning signs such as vital signs and central line days.
  • Communicate and collaborate effectively with patients, visitors, and clinical team members.
  • Report any variations in patient's clinical status or behavior promptly.
  • Document patient behavior and interventions, and escalate to clinical team for immediate response.
  • Suggest discharge from remote monitoring based on patient behavior.
  • Monitor Epic telehealth consult orders and ensure sessions begin and complete as scheduled.
  • Protect patient privacy by adhering to information security policies and HIPAA guidelines.
  • Escalate patient safety concerns and telehealth equipment issues promptly.
  • Maintain accurate records of interventions and safety events.
  • Set priorities, problem solve, and use proper judgment in difficult situations.
  • Submit utilization and assignment report every shift.
  • Maintain attendance/punctuality records and a safe, clean environment.
