Our client is seeking a compassionate and patient-focused Adherence Navigator to support individuals living with chronic and specialty medical conditions throughout their treatment journey. This role is ideal for a Social Worker or behavioral health professional who is passionate about patient advocacy, motivational coaching, and improving health outcomes through education and ongoing support.
Working remotely, you will engage directly with patients and caregivers by phone and other communication channels to provide education, address barriers to treatment adherence, encourage positive health behaviors, and empower patients to become active participants in their care. Key Responsibilities
Provide compassionate, high-quality support to patients and caregivers through outbound and inbound phone calls, email, text messaging, and other approved communication channels.
Educate patients on their diagnosed condition, prescribed therapies, and treatment expectations using approved educational materials.
Build trusting relationships through empathy, active listening, and patient-centered communication.
Identify and help patients overcome barriers to treatment adherence, including:
Medication concerns
Side effects
Financial challenges
Motivation
Caregiver support
Communication with healthcare providers
Utilize motivational interviewing and behavioral coaching techniques to encourage positive health outcomes.
Document all patient interactions accurately within a CRM or case management system.
Maintain detailed, timely, and compliant records while ensuring confidentiality and HIPAA compliance.
Recognize situations involving medical, legal, or financial concerns and escalate appropriately according to established protocols.
Participate in quality assurance initiatives and coaching sessions to maintain exceptional service standards.
Collaborate with internal teams to ensure patients receive seamless, high-quality support throughout their treatment journey.
Maintain productivity, quality, and attendance expectations in a remote work environment.
Qualifications
Required
Bachelor's degree in Social Work (BSW) or equivalent human services field.
Minimum of 2 years of professional experience in social work, patient support, counseling, or healthcare navigation.
Experience providing counseling, coaching, or patient support over the phone and/or virtual platforms.
Strong interpersonal, communication, and active listening skills.
Experience documenting interactions within CRM or case management systems.
Excellent organizational skills with strong attention to detail.
Ability to work independently in a remote environment while managing multiple priorities.
Dedicated, distraction-free home office with reliable internet service.
Preferred
Experience supporting specialty pharmacy or chronic disease patient populations.
Experience working with cardiovascular or other complex disease states.
Knowledge of motivational interviewing and behavioral coaching techniques.
Bilingual Spanish/English.
