Overview

The Vice President of Care Transitions, Home Health is responsible for the strategic leadership, development, and execution of the organization's Care Transitions program across home health operations. This role drives operational growth through hospital and senior living partnerships, embedded transition resources, and referral optimization strategies. The VP of Care Transitions defines and operationalizes the care transition model, ensuring scalable processes that support increased episodic admissions, institutional growth, and market expansion. This leader works cross-functionally with Business Development, Operations, Clinical, and Home Health Sales teams to strengthen referral pathways, enhance patient access, and improve care coordination and operational performance. Additionally, the VP serves as a key external representative to hospitals, health systems, and senior living partners, positioning the organization as a preferred provider for postacute care services.

Responsibilities

  • Develop and lead the national operational Care Transitions strategy, including program design, implementation, and continuous optimization.
  • Define and execute scalable models for embedding Care Transition resources within hospitals, senior living communities, and other referral sources to improve the care transitions process.
  • Drive episodic admission growth and institutional expansion through strategic partnerships, transitional process, operational excellence and improved care coordination.
  • Identify operational opportunities to improve care transitions, expand service capabilities and support strategic market growth.
  • Partner with Vice Presidents of Business Development and Home Health Sales teams to align operational growth strategies and effectively position Care Transitions resources.
  • Lead the development and standardization of Care Transitions training programs in coordination with Home Health Business Development Training Team.
  • Establish and monitor performance metrics, including admissions, referral conversion, and market share growth, ensuring achievement of organizational targets.
  • Analyze market data and referral trends to guide decision-making and optimize
  • Collaborate with Operations, Clinical, and Finance teams to support program implementation, resource allocation, and operational efficiency.
  • Represent the organization in executive-level meetings with hospitals and strategic partners to build preferred provider relationships and advance market presence.
  • Ensure high levels of customer satisfaction among referral sources, addressing issues and driving continuous improvement in care transition processes.

Qualifications

  • Bachelor's degree in Nursing, Therapy (PT/OT), Healthcare Administration, Business, or related field required; advanced degree preferred.
  • Active clinical license (RN, LPN, PT, OT) strongly preferred.
  • Minimum of 8-10 years of progressive leadership experience in home health, postacute care, or healthcare services.
  • Demonstrated success in business development, sales strategy, and driving measurable revenue and census growth.
  • Extensive experience building and maintaining relationships with hospitals, health systems, and senior living providers.
  • Proven track record overseeing or partnering with clinical liaisons, care transition teams, or referral development functions.
  • Strong knowledge of home health operations, regulatory requirements, and postacute care coordination.
  • Experience working in a matrixed organization, influencing cross-functional teams.
  • Excellent communication, analytical, and strategic planning skills, with the ability to operate at both executive and operational levels
