RN - Home Health FT Schedule: Monday to Friday Central Vermont Home Health & Hospice is more than just a healthcare provider; we're a community-focused, not-for-profit organization dedicated to enhancing the well-being of central Vermonters. In addition, this position serves as Case Manager in cases in which this position is the lead discipline.COVID VACCINATION REQUIRED - no exemptions.Oasis experience is a must.Home health experience is required.Experience as a home care traveler with proficiency in OASIS is preferred, as this role includes case management responsibilities.Homecare Home Base experience is preferred.
