This is an opportunity for an experienced home health nurse who enjoys the clinical side of admissions, values independence, and wants to earn additional income without committing to another traditional Monday through Friday nursing position.

Your Assessment Sets Everything In Motion

The Start of Care nurse has an incredibly important role in home health.

You are often the first clinician entering the patient's home. You'll assess their condition, identify immediate concerns, determine what services are needed, educate the patient and family, and establish the clinical foundation for the entire episode of care.

For an experienced home health nurse, it's an opportunity to use the full range of your assessment, OASIS, care coordination, and patient education skills.

What You'll Do

• Complete weekend Start of Care (SOC) admissions in patients' homes.
  
• Perform comprehensive nursing assessments and complete OASIS documentation.
  
• Develop and initiate individualized plans of care.
  
• Identify clinical, medication, safety, and disease-management needs.
  
• Educate patients and caregivers on medications, diagnoses, symptoms, and home care.
  
• Coordinate necessary services with nurses, therapists, physicians, and other members of the interdisciplinary team.
  
• Complete accurate and timely electronic documentation.
  

What We're Looking For

• Active Minnesota Registered Nurse (RN) license.
  
• Current CPR certification.
  
• Previous home health experience required.
  
• Strong working knowledge of OASIS and Start of Care admissions.
  
• Excellent clinical assessment and documentation skills.
  
• Confidence working independently in patients' homes.
  
• Valid driver's license, active auto insurance, and reliable transportation.
