Position Summary:We are seeking a compassionate and experienced
Registered Nurse (RN) to provide
weekend home health services across the
South County region. This role involves planning, organizing, and coordinating care, with a strong emphasis on community health and patient education.
Responsibilities:- Develop and initiate individualized plans of care in collaboration with patients, caregivers, and physicians
- Deliver preventative and rehabilitative nursing procedures as ordered
- Educate patients and caregivers on health management and care techniques
- Identify and implement discharge planning needs
- Coordinate necessary equipment and services for patient care
- Supervise LPNs/LVNs and Home Health Aides
Requirements:- Active California RN license
- Minimum 2 years of direct patient care
- Minimum 1 year of home health experience required
- Current CPR certification
- Valid driver's license and insured, reliable vehicle
