Job Description
A nonprofit organization serving supportive housing residents is seeking a Wellness Coach in the Bronx, NY. This role supports clients through case management, community resource coordination, and hands-on engagement within a scattered-site housing program.
About the Opportunity:
Shift: Day shift
Schedule: Sunday through Thursday or Tuesday through Saturday
Hours: 10am to 6pm
Setting: Onsite
Responsibilities:
Manage an assigned caseload and provide ongoing case management support
Develop, monitor, and update individualized service plans
Conduct home visits, community outreach, and follow-up with clients in scattered-site settings
Coordinate referrals and connect clients to medical, behavioral health, housing, employment, and public benefit services
Document services, progress notes, and client interactions in the assigned system
Qualifications:
3+ years of experience working with Behavioral Health populations
Bachelor's degree
Experience working in a Scattered-Site Supportive Housing setting
Working knowledge of AWARDS
Working knowledge of Microsoft Word, Excel, PowerPoint, email, and Windows-based systems
Desired Skills:
- Bilingual (English and Spanish)
Experience supporting individuals with mental health, substance use, or homelessness-related needs
Experience with case management, community outreach, group work, or tenant services
Familiarity with peer counseling, advocacy, or harm reduction approaches
