Job Description
A New York City nonprofit organization is seeking a Wellness Coach to join their Supportive Housing team in the Bronx. This role supports residents through case management, community resource coordination, and wellness-focused assistance in a busy onsite setting.
About the Opportunity:
- Shift: Day shift
- Schedule: Tuesday to Saturday or Sunday to Thursday
- Hours: 9:00 AM to 5:00 PM
- Setting: Onsite
Responsibilities:
- Provide case management support to an assigned caseload of residents
- Develop, monitor, and update individualized service plans
- Coordinate referrals and follow up for medical, behavioral health, vocational, and public benefit services
- Document progress notes, service contacts, and other required case records
- Support residents with community integration, groups, events, and daily living skill development
Qualifications:
- 3+ years of experience working with the Behavioral Health population
- Bachelor's Degree
- Experience in a Scattered Site Supportive Housing setting
- Working knowledge of AWARDS
- Working knowledge of Microsoft Word, Excel, PowerPoint, Windows, and email
Desired Skills:
- Bilingual (English/Spanish)
- Experience working with individuals impacted by Mental Illness, Substance Use, or Homelessness
- Experience with Case Management, Referral Coordination, Group Work, or Community Outreach
- Familiarity with Peer Counseling or Advocacy
