Job Description
A nonprofit services network in New York City is seeking a Wellness Coach to serve supportive housing communities in the Bronx. The role focuses on helping residents access community resources, manage care plans, and receive compassionate, hands-on support in a single-site setting.
About the Opportunity:
- Shift: Day
- Schedule: Tuesday to Saturday
- Hours: 10am to 6pm
- Setting: Onsite
Responsibilities:
- Provide case management support to an assigned caseload of residents
- Develop and monitor service plans in coordination with residents and care teams
- Assist with referrals to medical, behavioral health, vocational, and community resources
- Document progress notes and maintain accurate case records in the agency system
- Support resident engagement through groups, events, home visits, and ongoing follow-up
Qualifications:
- 3 years of experience working with Behavioral Health populations
- Bachelor's Degree
- Experience in a Scattered-site Supportive Housing setting
- Working knowledge of email, Microsoft Word, Excel, PowerPoint, and Windows operating systems
- Working knowledge of AWARDS
Desired Skills:
- Bilingual (English and Spanish)
- Experience supporting individuals with Mental Health and Substance Use needs
- Background in Case Management, Community Outreach, Referral Services, or Group Work
- Experience with Crisis Intervention, Supportive Counseling, or Peer Advocacy
- Familiarity with Medical Billing and Public Benefit Coordination
