A community health organization in Brooklyn, NY is seeking a Wellness Coach to provide hands-on support, case management, and wellness coordination in a team-oriented residential setting. This role focuses on helping participants access services, maintain daily living skills, and support continuity of care in a collaborative environment.

About the Opportunity:

  • Shift: Day shift

  • Schedule: Tuesday to Saturday

  • Hours: Either 8am to 4pm or 9am to 5pm

  • Setting: Onsite

Responsibilities:

  • Provide case management support to an assigned caseload of participants

  • Conduct wellness checks, inspections, and related follow-up in a residential setting

  • Assist participants with accessing community resources, referrals, and ongoing support services

  • Support daily living skills, documentation, and maintenance of apartment standards

  • Collaborate with team members on desk coverage, event setup, participant support, and other assigned duties

Qualifications:

  • High School Diploma with at least 4 years of demonstrated experience providing services to clients, or a Bachelor's Degree

  • Experience in Case management

  • Working knowledge of email, Microsoft Word, Excel, PowerPoint, and Windows operating systems

  • Ability to write case notes and maintain accurate documentation

  • Ability to support clients through community-based services, advocacy, and follow-up

Desired Skills:

  • Bilingual (English and Spanish)

  • Experience working with individuals affected by mental illness, substance use, or homelessness

  • Experience in supportive housing, group work, tenant organization, referral coordination, or community outreach

  • Familiarity with AWARDS

  • Background in behavioral health, medical follow-up, or HRA benefit coordination
