Growing together.Wellness Coordinatorsfunction in a non-clinical capacity responsible for providing care coordination or administrative support to members with behavioral health and substance abuse needs.Wellness Coordinatorswill be a primary care navigator by locating and engage members screening for and identifying gaps in care addressing SDOH needs and developing individual care plans to overcome barriers to access to care.Wellness Coordinatorswill also provide administrative support to the Behavioral Health Case Management department through assigning referrals providing chart summaries and verification of member demographics.Schedule Monday through Friday a 40-hour work week between the hours of 800 am to 500 pm CST.This is a field base position when not in the field will work from home.Location Communicable distance to Kansas City Missouri or Kansas City KansasIf you reside in Kansas City Missouri or Kansas City Kansas youll enjoy the flexibility to telecommute as you take on some tough challenges.Primary ResponsibilitiesWellness Coordinators work in an inbound and outbound member queueDocumentation in multiple electronic documentation systemsCoordinate information to clinical team partnersProvide behavioral health benefit educationContact members to screen for and identity gaps in careProvide network referrals and appropriate community resources to help the member engage with mental health and substance use treatmentAdministrative supportPerforms other duties as assignedWhat are the reasons to consider working for UnitedHealth GroupPut it all together - competitive base pay a full and comprehensive benefit program performance rewards and a management team who demonstrates their commitment to your success.
