Job Description

Schedule / Shift - Tues thru Sat; 9am-5:30pm
Payrate - $19.00-$20.00

Job Summary:

At MBK, we believe that every resident living with dementia has the capacity to experience joy, connection, purpose, and self-esteem—at every stage of their cognitive journey. The role of the Wellness Program Coordinator is to uncover and cultivate each resident’s potential to engage in these positive daily experiences and to grow. This goal is achieved by thoughtfully planning and delivering a dynamic daily Wellness Program using the structures, tools, and content provided by MBK, while adapting them creatively to meet the individual needs, abilities, and preferences of each resident.

Duties & Responsibilities:

  • Conduct the Lifestyle Survey interview with the resident and/or family (typically within 7 days of move-in) to identify interests, preferences, and potential engagement goals. Share the completed survey results and any goals with the Connections for Living Director (CFLD) and the Director of Health Services (DHS) for integration into the Service Plan or the 30-day service plan update, to support a smooth transition into wellness programs and the community.
  • Assist the team in finding more effective ways to meet residents’ needs when they express distress through verbal or nonverbal communication.
  • Plan and lead daily large group, small group, and individual experiences using the Daily Flow structure, incorporating residents’ interests from Lifestyle Surveys and service plan goals, and leveraging MBK-provided tools such as iN2L, Rendever, Eversound, SingFit, Montessori-Based Kits, and more.
  • Provide training and support to ensure that Wellness Programs are conducted when the Wellness Program Coordinator is not present, including evenings and days off.
  • Provide memory care team members with the training and supplies they need to build strong relationships, communicate effectively, and facilitate engaging individual, small, or large group programs to support residents’ greatest potential.
  • Perform administrative activities, including answering emails, attending required meetings and training, and contributing to shared projects, channels, and discussions.
  • Effectively manage program supplies and services within established monthly and annual budgets, minimizing waste and misuse of materials, and ensuring responsible, cost-conscious use of equipment and resources.
  • Support community sales and marketing by effectively showcasing wellness programs for potential residents, their families, and guests, and capturing engaging photos and videos to share on the community’s social media channels.
  • Maintain a safe and secure environment for all staff, residents, and guests following established safety standards, policies, and procedures.
  • Understand and comply with all Federal, State, and local regulations, and all company policies and procedures concerning the department.
  • Promote and lead with a spirit of teamwork and open communication in accordance with the MBK principles and core values.
  • Perform other job duties or special projects as assigned or requested by the Supervisor or Executive Director.

Education Requirements:

  • High school diploma or GED, required.
  • Bachelor’s Degree in psychology, social services, gerontology, social work, nursing, or recreational therapy, highly preferred.
  • Certified Dementia Practitioner (CDP) or similar certification, preferred.
  • Basic knowledge of Alzheimer’s Disease and other dementias, required.

Experience Requirements (in years):

  • 2+ years of prior work experience in a similar role supporting people living with dementia or other cognitive differences preferred.

Required Competencies/Licenses/Certifications:

  • Must have solid PC skills and be familiar with Microsoft Office Suite (Word, Excel, Outlook, Teams, etc.), and office equipment (e.g., scanners, copiers, and fax machines).
  • Must have excellent written and verbal communication skills, including the ability to speak, write, and read English.
  • Must possess the ability to empathize with others, understand their needs, and respond in a helpful way.
  • Must possess the ability to always deal tactfully and professionally with team members, residents, family members, and guests.

Physical Demands & Work Environment:

  • Must be able and willing to work a Tuesday – Saturday or Sunday – Thursday schedule.
  • Must be mobile and able to perform the physical requirements of the job, including bending, kneeling, squatting, pulling, reaching overhead, stacking chairs, and repetitive motion, including the ability to stand and walk for prolonged periods of time, and remain in a stationary position or move about for up to 8 hours.
