Job Summary:
We are seeking a highly skilled and compassionate Mobile Wound Care Nurse Practitioner (NP) to join our growing team. The ideal candidate will be responsible for evaluating, diagnosing, and treating patients with acute and chronic wounds, ensuring the highest level of care in a mobile setting. This role requires strong clinical judgment, autonomy, and excellent communication skills to collaborate with patients, caregivers, and interdisciplinary teams.
Key Responsibilities:
Provide comprehensive wound assessments and treatment plans for patients in their homes, skilled nursing facilities, and other care settings.
Perform debridement procedures (sharp, enzymatic, autolytic, mechanical) as necessary.
Utilize advanced wound care techniques, including negative pressure wound therapy, compression therapy, and biologic dressings.
Assess for and manage wound infections, collaborating with physicians for antibiotic therapy when needed.
Educate patients, caregivers, and facility staff on wound prevention, treatment, and management strategies.
Coordinate care with primary care providers, home health agencies, and other specialists to ensure continuity of care.
Document patient encounters in an EMR system in compliance with HIPAA and healthcare regulations.
Monitor patient progress and adjust treatment plans accordingly to achieve optimal healing outcomes.
Stay up to date with the latest evidence-based wound care practices and guidelines.
Qualifications:
Licensed Nurse Practitioner (NP) in Nevada.
Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology NP (AGNP), or other relevant certification.
Certified Wound Specialist (CWS), Wound Care Certification (WCC), or similar is preferred.
Minimum of 1-2 years of experience in wound care, dermatology, home health, or related specialty.
Strong ability to work independently and autonomously in the field.
Excellent communication and documentation skills.
Must have a valid driver’s license and reliable transportation for travel within the service area.
