We are seeking a compassionate and skilled Wound Care Nurse Practitioner to join our team providing specialized wound care services to patients in nursing home facilities across Central California. The ideal candidate will possess strong clinicalskills, excellent communication abilities, and a passion for geriatric care.
Available Positions
Full-Time Position: 5 days per week (40 hours)
Part-Time Position: 3 days per week (24 hours)
Key Responsibilities
Conduct comprehensive wound assessments and develop treatment plans for nursing home residents
Perform wound debridement, dressing changes, and other wound care procedures
Document patient conditions, treatments, and progress according to professional standards
Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
Educate nursing staff and caregivers on wound prevention and management
Follow up on patient progress and adjust treatment plans as needed
Travel to different nursing home facilities (one facility per day)
Manage a daily caseload of 20-30 patients
Qualifications
Current Nurse Practitioner license in California
Master's degree or higher in Nursing
At least one year of clinical experience as a Nurse Practitioner
Experience in wound care preferred but not required (no wound care certification necessary)
Excellent assessment, critical thinking, and decision-making skills
Strong interpersonal and communication abilities
Valid driver's license and reliable transportation
Work Schedule
8-hour workdays
One nursing home facility per day
Consistent assignment to specific cities to minimize travel variation
Location
Practice areas include Modesto, Merced, Sonora, and Madera, California
