Job Details

Wound Care Nurse Practitioner / Physician Assistant (Home Health)

We are hiring a Wound Care Nurse Practitioner / Physician Assistant to provide advanced wound care in patients’ homes throughout the Raleigh area.

This is a full-time home health / mobile wound care role for an NP or PA-C who values autonomy, strong patient relationships, and a predictable Monday–Friday schedule.

Responsibilities
Perform comprehensive wound assessments in the home
Follow prescribed wound care treatment plans
Monitor healing progress and adjust care within protocol
Educate patients and caregivers on wound management and prevention
Document visits accurately in the EHR
Coordinate care with physicians, home health teams, and other providers
Travel throughout the Raleigh territory for scheduled patient visits

Qualifications
Active Nurse Practitioner or Physician Assistant / PA-C license in North Carolina
Experience in wound care, home health, or mobile clinical care preferred
Strong assessment, documentation, and patient education skills
Able to work independently while following clinical protocols
Comfortable with field-based travel and in-home visits
