Wound Care Nurse Practitioner

A growing home-based Primary Care & Wound Care practice is seeking an experienced Wound Care Nurse Practitioner who is also comfortable managing primary care needs for a home-bound patient population on Chicago's South Side. This is a 1099 independent contractor position offering flexible scheduling and competitive per-visit/per-diem compensation plus mileage reimbursement.

You'll provide in-home wound care as your primary focus, along with general primary care management, working independently in the field with support from our clinical and administrative team.

Responsibilities:

• Serve as the lead clinician for chronic and complex wound management (sharp debridement, dressing changes, wound assessment/tracking, treatment planning)
• Provide primary care services during visits (chronic disease management, medication management, routine assessments)
• Complete accurate, timely documentation in our EMR
• Coordinate care with collaborating physicians, specialists, and family caregivers
• Manage a route-optimized daily caseload across South Side neighborhoods (e.g., Hyde Park, Chatham, Beverly, South Shore, Roseland)
• Order and interpret diagnostics as needed

Qualifications:

• Active Illinois APRN license in good standing
• Board certification (FNP, AGNP, or equivalent)
• Wound care experience required — sharp debridement experience strongly preferred
• Reliable transportation and valid driver's license (required for home visits)
• Comfortable working independently in a field-based role

Compensation:

• 1099 Independent Contractor
• Paid per visit or per diem — structure to be discussed based on caseload/schedule
