Job Summary
We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. Responsibilities
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Administer wound care and monitor patients for improvement or decline
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the Providers
- Utilize electronic health record (EHR) systems like WoundAlly for accurate documentation of patient care activities and medical records.
- Visiting patients home, facilities, etc. in the Los Angeles County, Orange County, Inland Empire and surrounding areas.
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid/CPR certification required
- Wound Care certificate (preferred)
- Proof of personal liability
- Car Insurance
