Responsibilities

  • Assess and document wound status and progress during on-site visits.

  • Develop and implement individualized wound care plans with measurable goals.

  • Provide evidence-based wound care treatments and dressing changes.

  • Adhere to infection control protocols and patient safety standards.

  • Monitor and document patient outcomes to track pipeline and targets.

  • Coordinate with interdisciplinary team to optimize care continuity.

  • Ensure compliance with regulatory standards and facility policies.

  • Maintain accurate patient records and secure reporting of progress metrics.

  • Demonstrate accountability for personal performance and meeting targets.

  • Educate patients and families on wound care and self-management.

  • Identify opportunities for professional development and skills advancement.


Experience/Qualifications

  • Active RN license with wound care certification preferred.

  • Proven clinical expertise in wound assessment and dressing changes.

  • Ability to develop and implement measurable wound care plans.

  • Strong documentation and electronic health record accuracy.

  • Infection control and patient safety protocol adherence.

  • Excellent coordination with interdisciplinary team for care continuity.

  • Knowledge of regulatory standards and facility policies.

  • Track patient outcomes and pipeline metrics for performance visibility.

  • Strong communication skills for patient and family education.
