Key Responsibilities:
Conduct comprehensive wound, ostomy, and continence assessments
Develop and implement individualized plans of care
Evaluate and document patient responses to treatment interventions
Provide expert consultation on complex wounds, fistulas, ostomies, and continence-related conditions
Perform pressure injury risk assessments and implement preventative measures
Educate patients, families, and staff on wound prevention and treatment strategies
Collaborate with physicians and multidisciplinary teams to optimize patient outcomes
Participate in quality improvement initiatives and nursing excellence programs
Requirements:
Active New York State Registered Nurse (RN) License
3–5 years of healthcare experience required
Strong clinical assessment and documentation skills
Excellent communication and interpersonal abilities
REQUIRED WOUND CARE CERTIFICATION (Must Have One of the Following):
Certified Wound Ostomy Nurse (CWON) – WOCNCB
Certified Wound Care Nurse (CWCN) – WOCNCB
Certified Wound Specialist (CWS) – ABWM
Wound Care Certified (WCC) – NAWCO
Candidates without an active wound care certification will not be considered.
Additional Details:
• Flexible hours to support patient-centered care
• Occasional holiday and/or weekend coverage may be required
• Experience utilizing electronic medical record systems preferred
