Job Summary/Objective:

Wraparound Coordination-Intensive Care is a community-based service within the youth services continuum.  Wraparound is a strengths-based approach aligning treatment providers and natural supports to help meet the needs of youth and families. Wraparound coordinators work closely with family members to build a Family Plan; facilitate cross system consultation and identify resources to meet the family’s needs. 

Essential Duties and Functions:

  • Completes psychosocial assessments inclusive of a diagnosis, case conceptualization, level of care assessment, and recommendations for treatment.
  • Engages in person-centered planning to develop goals and objectives.
  • Develops a comprehensive Wrap Plan which incorporates the needs of all family members in all domains.
  • Assists families in the identification of service providers and natural supports for their Family Team. 
  • Coordinates and facilitates Family Team meetings
  • Coordinates appropriate referrals to other youth-oriented programs such as Outpatient, Home-Based, MST, School Wellness, and/or Youth Peer Support Services.
  • Implements a customized set of strategies, supports and services to meet the individualized goals of the youth and family.
  • Conducts educational presentations.
  • Maintains record compliance according to agency guidelines and standards as well as funding source regulations. 
  • Ensures compliance and participation with competency improvement activities including, but not limited to credentialing, continuing education (per licensure requirements  or minimum  of  24 hours per year of clinically based trainings)

Qualifications:

  • Bachelor degree in human services related field, required.
  • One year specialized training in treating or working with a minor and his/her family who live with a severe emotional disturbance preferred. 
  • Licensure in the State of Michigan preferred, i.e. LLBSW/LBSW. 
  • One year of experience in a behavioral health care setting preferred.
  • Child Mental Health Professional qualifications (as defined by the Michigan Department of Health and Human Services) is preferred.
  • Demonstrates strong verbal and written communication skills.
  • One (1) year of computer experience – Word, Excel and electronic medical record software preferred
  • Not excluded from participating in Medicaid, Medicare, or any other federal or Michigan health care programs.
  • Must support a trauma informed culture of safety to aid customers in their recovery process.
