Job Summary and Responsibilities
As our Wraparound Facilitator, you will be a central pillar in empowering individuals and families, guiding them toward resilience and holistic well-being through coordinated, individualized support.
Every day, you will drive the development and implementation of highly individualized care plans by engaging natural supports, community resources, and professional services. You'll ensure these plans are person-centered, strengths-based, and culturally responsive, always aligning with the unique vision and needs of the individual and family. Your work will involve meticulous tracking of progress, proactive problem-solving, and continuous team collaboration.
To be successful in this impactful role, you will combine exceptional interpersonal and communication skills with a profound understanding of Wraparound principles, a solution-focused mindset, and the clarity to navigate complex systems while advocating for the best outcomes.

Founded in 1887, Dignity Health - California Hospital Medical Center is a 318-bed, acute care, nonprofit hospital located in downtown Los Angeles, California. Serving over 100,000 patients annually, the hospital offers a full complement of services including a Level II Trauma Center, Level III NICU, heart care, women's health, and a family birth center.
