Wraparound Facilitator / Multi-Systems Services Coordinator

Responsibilities include engaging youth, families, natural supports, and system partners in collaborative planning; monitoring service implementation and progress; identifying and addressing service gaps; coordinating re-entry planning for youth returning from out-of-home placements; maintaining timely documentation and case records; tracking outcomes and program data; and assisting families in accessing supportive funding and resources.

The successful candidate will demonstrate a commitment to family voice and choice, strength-based practice, community collaboration, and unconditional care. This position also serves as a liaison with community organizations and public agencies, participates in program evaluation and quality improvement efforts, attends required meetings and trainings, and supports the agency''s mission of improving outcomes for children and families.
