The Youth Transition Coordinator (YTC) partners with youth experiencing homelessness as they work to achieve safety, stability and permanency. YTCs are skillful in quickly establishing rapport with youth -- collaborating with them to develop a plan of care consisting of clear, measurable goals that not only address their housing needs but also serve to strengthening their family relationships, address mental and/or physical health needs, educational needs, and employment opportunities. The YTC also plays an active role in engaging houseless and runaway youth in conversations geared towards harm reduction, healthy relationships, overall support, and advocacy. YTCs participate in a variety of diversion activities, including family mediation and advocacy work within their assigned communities. MINIMUM QUALIFICATIONS: (includes knowledge, skills and abilities required as well as equipment not provided by agency, but necessary to perform job functions) EDUCATION, EXPERIENCE AND CERTIFICATESBachelor's degree with a minimum of one years' experience working in case management. Must be able to pass background checks including DMV, CPS, APS, and Criminal checks.
