As a key member of the hospice team, you will deliver quality end-of-life care in a supportive and empathetic manner, focusing on comfort, symptom management, and emotional support. You will work collaboratively with physicians, social workers, chaplains, and other healthcare professionals to ensure a dignified and peaceful transition for patients. Schedule: Full-Time, Weekdays, 8am - 5pm, On-Call  **Mileage Reimbursement** Primary Responsibilities and Duties: <ul><li> </li><li>Conduct thorough assessments of patients; physical, emotional, and spiritual needs.</li><li> </li><li>Develop and implement individualized care plans in collaboration with the patient, family, and other hospice staff.</li><li> </li><li>Administer medications, manage pain and symptom control, and perform medical procedures as needed.</li><li> </li><li>Monitor and document patient progress, changes in condition, and response to treatments.</li><li> </li><li>Educate patients and their families on disease progression, care options, and symptom management.</li><li> </li><li>Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care.</li><li> </li><li>Offer emotional and psychological support to patients and families, addressing their concerns and facilitating discussions about end-of life care.</li><li> </li><li>Coordinate with other members of the hospice team to ensure comprehensive and continuous care.</li><li> </li><li>Advocate for patients’ preferences and needs, respecting their values and wishes.</li><li> </li><li>Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.</li><li> </li><li>Participate in on-call rotation as required to provide care and support after hours.</li><li> </li><li>Perform all other duties assigned.</li><li> </li></ul> </div> <div>Requirements</div> <div> Job Qualifications: <ul><li> </li><li>Valid license as a Registered Nurse (RN) in the state where work is performed.</li><li> </li><li>2 or more years of nursing experience, preferably with a focus on hospice, palliative care, or related fields.</li><li> </li><li>Strong clinical skills and knowledge of nursing principles, procedures, and best practices in long-term care.</li><li> </li><li>Excellent communication and interpersonal skills, with the ability to interact effectively with residents, families, caregivers, and healthcare professionals.</li><li> </li><li>Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.</li><li> </li><li>Understanding of regulatory requirements in hospice settings and nursing practices.</li><li> </li><li>Ability to build rapport with patients through a compassionate and caring demeanor.</li><li> </li></ul> Physical Requirements: <ul><li> </li><li>Ability to lift and transfer patients, with or without assistance, up to 50lbs.</li><li> </li><li>Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods.</li><li> </li><li>Strength to handle medical equipment and assist patients with mobility and physical activities.</li><li> </li><li>Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling delicate medical instruments.</li><li> </li><li>Ability to respond swiftly and efficiently to emergency situations.</li><li> </li><li>Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces.</li><li> </li><li>Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.</li><li> </li><li>The job requires accepting criticism and dealing calmly and effectively with high-stress situations.</li><li> </li><li>The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.</li><li> </li><li>The ability to keep your hand and arm steady while moving your arm or while holding your arm and hand in one position.</li><li> </li></ul> </div> <div>Salary Description</div> <div>$90,000 - $100,000 Per Year</div> </div>
