The Comprehensive Assessor serves as a key link between consumers and essential services, providing education on long-term care options, facilitating referrals, and coordinating with internal teams and community partners to ensure timely and appropriate service delivery. This role is responsible for conducting in-depth, multi-dimensional assessments to evaluate physical, functional, social, and environmental needs, determine eligibility for Medicaid and waiver programs, and develop person-centered care plans.
