ACT services are available to the client 24/7 are provided by a multidisciplinary team under the direction of an LPHA, and include the following rehabilitative services: 1) All Mental Health services funded by Medicaid that an MHP-level staff member can provide, 2) crisis services, 3) medication services, and 4) individual and group therapy 5) all Case Management services. Responsibilities include providing oversight of the evaluation process, treatment planning, resource development and any other task needed to ensure adequate supports are in place so clients will remain safely and successfully in the community.
