We are looking to add a Value Based Care Coordinator to perform various duties to support our growing infrastructure. The ideal Value Based Care Coordinator will be expected to prioritize and balance tasks while working in a fast-paced environment.Job Role: Reports to the Director of Business Development and is responsible for coordinating and supporting the value-based care and population health initiatives of the company.
