Advanced Practice Provider-Days-Full Time and Part time <ul><li>Salary Range: $ 102,613/yr - $159,050/yr based on full-time work and based on relevant years of experience. </li><li>Location: Larimer county, Colorado </li><li>Status: Full Time </li></ul>The salary range for this position is SummitStone's good faith belief and reasonable estimate of the range of possible compensation for this position at the time of the posting. Role Overview: An Acute Care Medical Provider is needed for day shift to help us in our endeavor to build the healthiest community in the nation ... one person at a time. This position will be based at the new and innovative acute care behavioral health facility, which opened in late 2023. SummitStone has the privilege of partnering closely with Larimer County on this venture. We provide comprehensive and collaborative care to individuals in crisis with mental health and/or substance use disorders. The advanced practice provider (PA or NP) will provide professional services in the evaluation and treatment of clients at our acute care facility. The primary focus will be rounding on clients admitted to our crisis and/or detox units. Major responsibilities of this position include: function as the medical team representative to a multidisciplinary treatment team designed to offer comprehensive behavioral health services; evaluate patients for substance use, mental health, and medical disorders, and determine the appropriate course of treatment for the patients; ability to thrive in a multidisciplinary team setting; meet/maintain documentation standards; and comply with county, state and federal laws, rules and regulations pertaining to health care. This position requires knowledge of diversity and inclusion with the relevant ability to engage, communicate, interact, and work effectively and cooperatively with people of different backgrounds, identities, and culturally diverse communities. Minimum Qualifications: <ul><li>Advanced Practice Nursing degree (APN) degree or Physician Assistant (PA) from an accredited university. </li><li>Demonstrated commitment and experience working with persons from marginalized communities and other patient populations experiencing vulnerabilities. </li><li>Understanding and commitment to Medications for Addiction Treatment and harm reduction principles. </li></ul>Preferred Qualifications:<ul><li>Spanish language skills. Rehabilitation Counseling Undergraduate Intern <ul><li>Location: Fort Collins, CO (Spirit Crossing Clubhouse) </li></ul>SummitStone Health Partners is seeking undergraduate-level Clinical Interns for the Fall 2026, Spring 2027, and Summer 2027semesters. This internship provides pre-professional experience in a co-occurring behavioral health setting, offering rich experience with both mental health and substance use disorder treatment. Internships are unpaid and designed to meet field placement requirements for accredited undergraduate programs. What You'll Do Assistance and guidance to adults with behavioral health disorders; Clubhouse members and SummitStone Health Partners consumers, to achieve vocational and daily living goals. Primarily functions as a Clubhouse generalist staff working in partnership with clubhouse members and staff in all areas of the Clubhouse program. Duties include both group and individual services through the coordination of activities and work tasks within the Clubhouse day program. Essential Responsibilities<ul><li>Build relationships with a variety of individuals </li><li>Advocate for individuals and the clubhouse program to the community at large for social and employment opportunities </li><li>Individual service may include goal setting, program planning, job coaching or supported education tutoring </li><li>Opportunity for on-going program evaluation to maintain program fidelity of Clubhouse Standards are offered </li><li>Documentation using electronic medical records system for group services and provision of basic case management services.
