Under the direct supervision of the Housing Program Manager, the Housing Community Supports Specialist will work collaboratively with internal resources, including the Long Term Services and Supports, Clinical, Behavioral Health, Enhanced Care Management (ECM), Health Care Services, and Case Management teams, as well as external resources, such as the Alameda County Continuum of Care team (Coordinated Entry), Alameda County Health, and other community-based organizations (CBOs) to provide social service linkage to members who demonstrate needing referrals to community, county, health, and social services programs. MINIMUM YEARS OF ADDITIONAL RELATED EXPERIENCE: Three - Five years' experience working in Permanent Supportive Housing, Rapid Rehousing, Transitional Housing, Affordable Housing, or Long-Term Care, while also providing supportive services within various housing models.
