This role guides individuals through the PACE intake and enrollment process by providing education about the PACE model, explaining eligibility requirements, conducting initial screenings, coordinating assessments, supporting completion of enrollment documentation, and facilitating timely handoffs to clinical, eligibility, outreach, and interdisciplinary team members. The Intake and Enrollment Specialist must be able to communicate clearly and compassionately, maintain accurate and timely documentation, manage multiple priorities, and support prospective participants and caregivers through a process that can be complex and highly personal.
