You will: • Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care • Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents • Obtain medical history, current medications, and symptoms • Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines • Coordinate the start of services (nursing, aides, equipment, medications.) • Graduate of an accredited nursing school with current licensure to practice in state of operation • One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
