Position SummaryThe Plover Place Housing Advocate supports adult residents by assisting with living skills responding to needs and crises and helping maintain a safe and respectful environment. The role also manages building access answers calls documents interactions and provides general administrative and event support. Plover Place is a 24-unit site-based permanent supportive housing program serving single adults experiencing or at risk of homelessness. Designed for efficiency and low maintenance it provides housing case management staffing and life skills programming to support residents in achieving stability.As part of The Salvation Army Northern Division Duluth Corps Plover Place operates within a broader mission of Doing the Most Good" by delivering holistic person-centered services. Documentation & Administrative Support 15 Perform administrative tasks including documenting participant interactions incidents and visitor logs. Caller & Information Requests 10 Respond to incoming calls and provide information or support as needed. Seasonal & Special Project Support 10 Assist with seasonal and special projects and events.Education and ExperienceHigh School diploma or equivalent required. Must be at least 18 years of age. Must have at least one year of experience with the target population served.Must pass background checks including through The Salvation Army and MN Department of Human Services.CompetenciesFlexible interpersonal and communication skills both verbal and written.Good organizational skills and attention to detail. Our Care Coordinators provide care coordination and case management services to individuals of all ages with disabilities, mental health conditions, or terminal illness. Using person-centered practices, they coordinate services for individuals with disabilities among different health and social service professionals and across settings of care. This is a part-time position working 20 hours per week.Accountabilities:<ul><li>Person-centered service delivery </li><li>Documentation/meet billing standards</li><li>Manage stakeholder relationships </li><li>Achieve quality and work standards </li></ul>Job Purpose and Reporting Structure:<ul><li>Provide care coordination services to individuals of all ages with disabilities, mental health conditions, or terminal illness. </li><li>Using person-centered practices, coordinate services for individuals with disabilities among different health and social service professionals and across settings of care. </li><li>Position reports to the Associate Director of Contracted Case Management. </li></ul>Essential Responsibilities:<ul><li>Listen-Learn-Lead: Model the mission and values of Accord, helping people to live their greatest lives.</li><li>Using person-centered practices, coordinate services for individuals with disabilities among different health and social service professionals and across settings of care. </li><li>Interact professionally with interdisciplinary team members including the person supported, family caregivers and guardians, employers, case managers, community providers, and other involved stakeholders.</li><li>Meet remotely and face-to-face and with individuals and their support teams to assess physical and mental wellness, needs, preferences, and abilities; apply information to develop tailored treatment plans. </li><li>Integrate and improve the coordination of care by: </li><li><ul><li>Ensuring optimal health status and decreasing the rate of health decline.</li><li>Providing social or community support systems.</li><li>Promoting a safe environment.</li><li>Reducing the impact of behavioral health issues.</li><li>Encouraging self-reliance.</li></ul></li><li>Provide Care Coordination services to assess, plan, facilitate and advocate for options and services to meet an individual’s health needs, through communication and available resources to promote high-quality, cost-effective health outcomes. </li><li>Advocate for individuals, including awareness of and sensitivity to culturally appropriate care. </li><li>Apply ethical practice principles such as respect for the autonomy, dignity, privacy and rights of the individual. </li><li>Promote collaborative, proactive, and person-focused relationships. </li><li>Provide timely and beneficial services that promote quality and cost-effective health outcomes. </li><li>Manage a flexible caseload of up to 60-80 members. </li><li>Attend all required annual and other meetings, demonstrating a high level of professionalism and genuine care for the person supported.</li><li>Practice customer service excellence and prompt responses (within one business day)  for all communication, including written and verbal correspondence.</li><li>Demonstrate strong written communication skills; complete all required paperwork to comply with contractors and funding sources.</li><li>Complete documentation of all services provided in the selected electronic health record/database system within one business day of providing services.</li><li>Meet compliance standards set forth by the managed care organizations standards.</li><li>As a mandated reporter, respond and report all incidents that occur while providing services to protect the health and safety of and minimize risk of harm to the person(s) supported following Accord’s policy and MN Statutes.</li><li>Represent the agency to the people we support, our community partners and external constituents.  Serve as a positive role model for other employees.</li><li>Meet completion deadlines for all required and assigned training related to the position. </li><li>Comply with all federal and state laws, regulations and administrative rules that apply to program areas.</li><li>Travel throughout St. Louis County and surrounding counties to job sites and meeting locations; limited overnight travel possible.</li><li>Perform other duties as assigned. </li></ul>Requirements<h3>Credentials/Qualifications:</h3>Must live in St. Louis County or surrounding counties.
