This includes assisting families in identifying strengths and needs, connecting them to agency and community resources, and fostering their confidence as parents and community members. The duties identified below are the essential functions of the position. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.<ul><li>Engage families in program services and activities that promote school readiness, parenting skills, knowledge, and confidence</li><li>Assist families in establishing and pursuing goals that enhance personal growth and connections to the community</li><li>Organize events in response to family needs and interests</li><li>Deliver relationship-based home visits and enrollment meetings that are responsive to family needs</li><li>Provide families with resource information and referrals to agency and community supports as needed</li><li>Complete monthly reports, documentation, and track family progress</li><li>Ensure compliance with Bauer Policies, State Licensing, and Head Start Program Performance Standards</li><li>Attend twice-monthly staff meetings, monthly Professional Development Days, quarterly All-Agency meetings, and annual Pre-Service</li></ul><h1>QUALIFICATIONS:</h1>To perform this position successfully, an individual must demonstrate regular, predictable attendance and be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge and skill required. WHAT WE'RE LOOKING FOR:
The Director of Social Services is primarily responsible for the implementation of social care plans and resident assessments to ensure that the medically related emotional and social needs of the residents are met and maintained on an individual basis.  Key Responsibilities<ul><li>Plan, develop and organize the social service programs of the Health Campus.</li><li>Participate in discharge planning, development and implementation of social care plans and resident assessments.</li><li>Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.</li><li>Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available to the residents.</li><li>Participate in IHCC allowed activities such as the development and oversight of a healthcare support plan that includes coordination of medical care and proactive care management of both chronic diseases and complex conditions such as recurring falls, depression and dementia. Assist in transitional care planning from hospital or nursing facility to home/assisted living.</li><li>Coordinate outside support services for residents to meet their care needs at home or in the health campus.</li></ul>Qualifications for Director of Social Services I: Indiana locations<ul><li>Bachelor's or advanced degree in Social Work or a Human Services field, including but not limited to: <ul><li>Special Education</li><li>Rehabilitation Counseling</li><li>Psychology</li></ul></li><li>Three (3) to five (5) years’ experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment</li></ul>MI, KY, OH, WI locations<ul><li>High school diploma or equivalent</li><li>Bachelor’s in Psychology; or</li><li>Licensed practical nurse in Kentucky, Michigan, or Ohio</li><li>Must meet State requirements and regulatory code(s) for licensure or certification</li><li>Three (3) to five (5) years’ experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment</li></ul>Qualifications for Director of Social Services II:<ul><li>Bachelor's degree in social work</li><li>LSW preferred</li><li>Must meet State requirements and regulatory code(s) for licensure or certification</li><li>Three (3) to five (5) years’ experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment</li></ul>Compensation will be determined based on the relevant license or education held, as well as the candidate’s years of experience
