We are currently accepting application for a full-time Social Services position in Lake Charles, LA.Lake Charles Care Center, a 182-bed skilled nursing facility, is seeking a Social Services Professional with long-term care experience preferred. The ideal candidate will demonstrate the ability to work in a fast-paced environment and direct the functions and activities of the nursing staff to deliver exceptional patient care. Our team members will count on you to provide direction and leadership.<ul><li>Full-Time</li><li>Monday - Friday (8a - 5p) hours may vary</li><li>Salary depends on experience</li></ul><ul><li>Work Location: In person</li></ul>In this position, you will be responsible for implementing social services in accordance with federal, state, and local standards, as well as established facility policies and procedures. This position ensures that the medical, emotional and social needs of the residents are met and maintained on an individual basis.Responsibilities:<ul><li>Work directly with residents, the care team, and family members to identify unique psychosocial, mental, and emotional needs</li><li>Complete psychosocial assessments</li><li>Develop and implement viable care plans</li><li>Attend care conferences</li><li>Work directly with other members of the residents' care team, such as hospice providers</li><li>Talk with families about complaints</li><li>Coordinate discharge planning and refer residents to appropriate services, such as home care or hospice</li></ul>Experience:<ul><li>Bachelor's in social work or in a related human services field including, but not limited to, sociology, rehabilitation counseling and/or psychology.</li><li>One year of supervised social work experience in a health care setting working directly with individuals; or a similar professional degree in a field such as counseling, special education, sociology or psychology.</li><li>Experience in long term care preferred</li><li>Ability to multitask</li><li>Familiarity of admissions and ability to work with business office and nursing to make sure residents are proper candidates for admission</li></ul>Competitive salary and benefits.
