Family Service Rochester is seeking a compassionate, community-focused Family Advocate to join our Family Resource Center (FRC) team in a full-time role. As a Family Advocate, you will help plan, coordinate, and deliver programs and services that empower families and promote child and family well-being. You will collaborate closely with families, kinship caregivers, community partners, stakeholders, and service providers to ensure families receive meaningful, accessible, and coordinated support. This role includes supporting grandparents, relatives, and other caregivers who are raising children through kinship care arrangements by helping them navigate systems, access resources, build support networks, and strengthen family stability. Position: Social Worker
Responsibilities:
In this role, the Social Worker will support nursing home residents on an as-needed basis by promoting their overall well-being and addressing emotional and psychosocial needs. This includes conducting interviews and assessments to identify individual needs, developing resident-centered care plans, and collaborating with staff to implement and sustain interventions that enhance quality of life. The Social Worker will also provide coverage and assistance as scheduled or when needed.
Duties:
<ul><li style="text-align:justify">Provide Social Services oversight, completing duties as assigned for the residents of Eden Senior Care.
</li><li style="text-align:justify">Work closely with the Marketing Director to coordinate the referral-to-admission process.
</li><li style="text-align:justify">Communicate with insurance companies to obtain prior authorization for admissions; keep a record of insurance updates and communicate with departments included in updates to ensure it is done in a timely.
</li><li style="text-align:justify">Assist new residents and family members with the completion of the Admission Paperwork.
</li><li style="text-align:justify">Provide information to residents/families regarding Medicare, Medicaid, and other financial assistance programs available to the resident; assist with completion of Medicaid applications as deemed necessary.
</li><li style="text-align:justify">Ensure PASRR is completed to state requirements.
</li><li style="text-align:justify">Participate in resident assessment – provide background information and social history, as identified in the MDS process, in accordance with the RAI manual.
</li><li style="text-align:justify">Involve the resident and family members in planning objectives and goals for the resident; develop a written plan of care to work towards meeting identified goals.
</li><li style="text-align:justify">Attend and lead Behavior Meetings and monitor Gradual Dose Reductions (GDR) and consent for psychotropic medications according to state regulations.
</li><li style="text-align:justify">Complete timely progress notes on each resident.
</li><li style="text-align:justify">Have an open-door policy to all residents, family members, and staff to listen to their problems or needs; do effective counseling or referrals to meet their needs.
</li><li style="text-align:justify">Report concerns/grievances to the Executive Director and initiate an investigation
</li><li style="text-align:justify">Serve as an advocate for the resident and his/her rights, helping the resident learn assertiveness in communicating their needs, wants, and problems to the appropriate departments and agencies.
</li><li style="text-align:justify">Participate in Monthly QAPI Committee meetings.
</li><li style="text-align:justify">Attend Stand-Up meetings daily; report to IDT about pay changes, care conferences, concerns, etc.
</li><li style="text-align:justify">Initiate and lead the discharge planning process; coordinate with outside resources such as DME, Home Health, Hospice, etc., to ensure a safe transition to home or lower care setting.
</li><li style="text-align:justify">Issue denial letters according to state regulations
</li><li style="text-align:justify">Assist in providing in-service education and orientation for employees regarding Resident Rights and the social service needs of residents
</li><li style="text-align:justify">All other duties assigned by the Administrator
</li></ul>Requirements:
<ul><li style="text-align:justify">Preferred: Bachelor’s degree in Social Work or a Bachelor’s degree in a Human Services field, including but not limited to Social Services, Sociology, Special Education, Rehabilitation Counseling, and Psychology.
</li><li style="text-align:justify">Preferred: Licensed Social Work certificate in the state of Minnesota, current and valid. (LSW)
</li><li style="text-align:justify">One year of supervised social work experience in a health care setting, working directly with individuals, is preferred but not required.
</li><li style="text-align:justify">Able to promote cooperation between social services and other department employees.
</li><li style="text-align:justify">Must be able to be professional over the phone and in person when representing the company.
