South Territory: Blue Island, Calumet City, Dolton, Flossmoor, Glenwood, Harvey, Markham, Hazel Crest, Homewood, Lansing, Matteson, Midlothian, Olympia Fields, Orland Park, Park Forest, Richton Park, South Holland, Thornton, Tinley Park, Country Club Hills, Alsip, Evergreen Park, Riverdale, and south Chicago. The Occupational Therapist will evaluate patients’ functional abilities, establish individualized plans of care, provide skilled occupational therapy, supervise assigned COTAs and Home Health Aides, and help patients safely perform daily activities in their home environment.
