DescriptionTEMPORARY/Full Time - Patient NavigatorBAART Programs is looking for an adaptable and effective Patient Navigator, SOR 3 Hub and Spoke Grant, to work with external entities including but not limited to: jails, hospital EDs, OBOTs, residential programs, other locations identified in need of help with outreach and education, to help identify persons with OUD in these settings and discuss referrals to MAT treatment and develop realistic individualized action plans for patients. Support patient to access the highest level of care available. Essential Duties & Responsibilities:  <ul><li>Assist TCD to develop outreach and education list.</li><li>Provide education and outreach to external entities developed mutually with TCD.</li><li>Round in ED, jail and other settings routinely during weekdays: check in with other settings to see if there are pts eligible for MAT.</li><li>Maintain required data for referrals provided as well as outreach/education activities engaged in.</li><li>Available on-call for MAT linkage during regular hours if not present in ED or jail</li><li>Describe program and principles of OUD MAT to external entities</li><li>Obtain consent for participation and complete MAT intake forms</li><li>Assess readiness using the ‘Stages of Change Model’</li><li>Assess for co-occurring substance use or psychiatric disorders and/or concurrent medical conditions</li><li>Confirm and/or assist to establish insurance status</li><li>If uninsured, connect patient with financial counseling service</li><li>Identify resources for OUD MAT, assist with making follow up MAT appointments</li><li>Assess and address any barriers to attending follow up appointments (transportation, contact phone # availability for appt reminders, shelter needs, etc). Engage social services to assist with any of these issues as needed.</li><li>Communicate with ED jail providers to inform them of MAT follow up plan including scheduled appointments.</li><li>After discharge from ED or release from jail, on the day prior to follow up appointment contact patients to remind them of their follow up appt.</li><li>Ability to handle stressful situations and interact with others using excellent communication techniques.</li><li>Must be present during working hours at the office for in person meetings and access to a computer without violating company policy.</li><li>Other duties, as assigned</li></ul> Qualifications: <ul><li>Nonjudgmental, energetic, positive approach to assisting patients with SUD</li><li>Understanding of SUD as a disease and the role of MAT in OUD treatment.</li><li>Exceptional oral and written communication skills are required. DUTIES:<ul><li>Works within an established framework to protect people, property, assets and interests.</li><li>Responsible for coordinating activities and resources in support of programs or projects that impact multiple stakeholders, systems, or workflows with moderate to high risk and complexity.</li><li>Responsible for identifying, defining, and documenting physical security risks, training deficiencies, and opportunities relative to physical security best practices and regulatory requirements related to the healthcare industry.</li><li>Compiles data, writes routine reports, and conducts training designed to ensure compliance and improve enterprise-wide security posture.</li><li>Coordinates and maintains physical security practices designed to safeguard personnel and prevent unauthorized access to acute and non-acute care facilities.</li><li>These Principal Accountabilities, Requirements and Qualifications are not exhaustive, but are merely the most descriptive of the current job.</li><li>Management reserves the right to revise the job description or require that other tasks be performed when the circumstances of the job change (for example, emergencies, staff changes, workload, or technical development).</li></ul> JOB ACCOUNTABILITIES:PROGRAM ADMINISTRATION:<ul><li>Coordinates the timely completion of annual security risk surveys and assessment reports.•</li><li>Develop, coordinate, and administer departmental programs, projects, and initiatives to reduce risk/crime.</li><li>Prepares security-related project plans, security risk assessments, and other documentation as required.</li><li>Serves as system or database administrator for various departmental applications.• Assist in project documentation management.• Conducts resource assignment and schedule maintenance.</li><li>Accepts other assignments for specialty projects as requested and deemed appropriate by assigned Manager</li><li>Files documents, posts deliverables, schedules meetings, and prepares agendas.</li></ul> DATA COLLECTION, MANAGEMENT, ANALYSIS, AND REPORTING:<ul><li>Compiles data and runs reports in support of security analyses and planning.</li><li>Evaluates the effectiveness of existing security practices and provides recommendations.•</li><li>Assists in incident trending, analyses, and report/presentation development as needed.• Assists with development of data collection/management and report automation programs.</li><li>Assists in the day-to-day management of Protective Services incident and daily reporting processes.</li><li>Works closely with other team members in collecting and routing information.• Utilizes electronic survey tools to obtain feedback and conduct needs assessments.</li></ul> TRAINING:<ul><li>Assists with training on crime prevention techniques, security awareness, workplace violence prevention, and various outreach programs</li><li>Develops training materials and conducts training for new programs, applications, and procedures.</li><li>Assure personnel adhere to established policies and practices while providing training where necessary</li></ul> SUPPORT DEPARTMENTAL INITIATIVES:<ul><li>Coordinate multiple concurrent projects and oversee them through completion.</li><li>Contributes to compliance with federal and state security and risk regulations including The Joint Commission, CMS, HIPAA, and Cal/OSHA; other state laws related to Workplace Violence prevention and healthcare security; and the organization’s protective services-related policies and procedures.</li><li>Collaborates with counterparts in the healthcare security industry to benchmark company performance and exchange best practices.</li><li>Builds and maintains effective working relationships with others</li></ul> REQUIREMENTS:<ul><li>Equivalent experience will be accepted in lieu of the required degree or diploma.</li><li>Bachelor's: healthcare, business, or related field or equivalent education/experience</li><li>2+ years of recent relevant experience.</li><li>Written and verbal communication skills.</li><li>Demonstrated success working in a highly regulated work environment.</li><li>Ability to accurately analyze situations in a highly complex organizational structure and reach appropriate decisions based on informed judgment.</li><li>Must be highly organized and detail oriented.Demonstrated ability to develop programs, processes, and procedures in collaboration with multiple cross-functional work groups and tailor approaches to highly diverse organizational structures and situations.Interpersonal and collaboration skills.</li><li>Must demonstrate a high level of integrity and professionalism to handle sensitive and confidential information.</li><li>Demonstrated ability to set priorities and respond to changing demands from multiple sources in a fast-paced environment.</li><li>Demonstrated ability to follow through, meet deadlines, anticipate requirements, and build relationships with minimal oversight.</li><li>Analytical, problem-solving skills, and decision-making skills.</li><li>Demonstrated ability to exercise independent judgment and employ reasoning skills.</li><li>Must be able to work collaboratively in a team environment with minimal supervision.</li><li>Time management and organization skills.</li><li>PC skills including MS Word, Excel, Outlook, Project, Visio, and PowerPoint.</li><li>Ability to travel on short notice and during non-standard working hours.</li></ul>Kavaliro provides Equal Employment Opportunities to all employees and applicants.
