We invite you to consider our per diem opportunity to earn supplemental income as an Independent Contracted Nurse. The assessments are completed face to face in the claimant's homes.This is a 1099 contract position to earn supplemental income. You will need a portable computer with Windows or tablet with camera.In this role, you will complete in-depth cognitive and functional assessments with clients in their homes.You will complete structured assessments, focusing on accuracy and efficiency.You will be focused on observing and reporting the current status of the individual. You must remain objective.You will coordinate all scheduling with client or their point of contact using contact information provided to you.You will submit an assessment within 24 hours through an online application. Upon submission, our review team will reach out with any needed clarifications by telephone within 48 hours. An assessment is not considered complete until all clarifications are answered.You will be paid $125 per assessment within a 35 mile radius, upon completion of each assessment.Requirements• A friendly, compassionate, dedicated, and patient personality.

We are hiring an as needed (PRN) Behavioral Health Assessor for our growing team in our in-patient psychiatric hospital division!  The Behavioral Health Assessor provides in-person risk assessment and crisis intervention, initial contact with referral sources and potential patients, coordinates the referral and intake process, determines the appropriate level of care, facilitates the admission process, and maintains communication with referral sources, families, patients, and unit staff. **Open to RNs, Clinical Counselors, and Social Workers Key Responsibilities:<ul><li style="list-style-type:disc">Completes comprehensive mental health and substance use assessments.</li><li style="list-style-type:disc">Identifies patient crisis needs.</li><li style="list-style-type:disc">Participates in ongoing assessment and care plan recommendations for patients with extended time awaiting dispositions. </li><li style="list-style-type:disc"> Provides crisis intervention services using a therapeutic intervention, which is based on patient-centered outcomes, and standards of care, that is based on current research and evidence-based practice. </li><li style="list-style-type:disc">Supplies information to the medical team for consideration of a differential diagnosis. </li><li style="list-style-type:disc">Pursues ongoing education in behavioral health topics. </li><li style="list-style-type:disc">Recognizes significant changes in patient condition and responds appropriately. </li><li style="list-style-type:disc">Navigates insurance verification and pre-certification processes for behavioral health carve-outs. </li><li style="list-style-type:disc">Provides psycho-education to family and significant others of patients, as well as other co-working caregivers about behavioral health, de-escalation, and/or other related topics to assist in the patient’s current plan of care. </li><li style="list-style-type:disc">Involves the patient/family (or designee) and other current providers in developing a plan of care individualized to the patient’s cultural, religious, and educational needs. </li><li style="list-style-type:disc">Collaborates with a multidisciplinary team in the formulation of an individualized plan of care. </li><li style="list-style-type:disc">Develops appropriate patient goals for identified problems. </li><li style="list-style-type:disc">Serves as a patient advocate. </li><li style="list-style-type:disc">Process referred cases to appropriate disposition; Overview:
Responsible to meet with Persons Served/Family in their homes to evaluate and assess the needs of the Person Served.  Schedule timely follow up visits to ensure provision of services by the assigned Direct Support Professional.  Bring and complete all necessary paperwork to the home visit with the Person Served.  Relay Person Served information to the Coordinator and the Support Services Manager. This role is ideal for a Certified Nursing Assistant (CNA) with home care experience who enjoys client interaction, assessment, and care coordination.

External Job Description:
<ul><li>Routinely schedules Home Visits per company/contract specifications</li><li>Completes all necessary documentation for Home Visits per specified contract</li><li>Assist Person Served with any issues or questions relayed during Home Visit</li><li>Reports issues, concerns and problems to Coordinator and Program Director</li><li>Enter Home Visit update information in correct systems</li><li>Document and track all Home Visit contact information with Person Served</li><li>Maintains on going communication with Persons Served, Coordinators and Program Director</li><li>Support Coordinators when requested by Supervisor</li><li>Reports significant/major incidents or accidents</li><li>Cooperates in external/internal investigations as requested</li><li>Other duties as assigned</li></ul>

Qualifications:
<ul><li>High School Diploma; two years supervisory experience or 1 year supervisory experience with one year of college.  Bachelors degree preferred</li><li>Valid drivers license from state of residence with a satisfactory driving record as defined by Company's vehicle policy and/or liability insurance carrier.
