As part of the clinic you will also provide dedicated clinical services during part of your clinical time to our heart failure clinic which is a highly tailored, specialty center focused on providing best in class heart failure management including optimizing guideline directed medical therapy, providing health coaching, and managing volume overload. Therapeutic Mentoring services include supporting, coaching, and training the youth in age-appropriate behaviors, interpersonal communication, problem-solving and conflict resolution, and relating appropriately to other children and adolescents, as well as adults. The Therapeutic Mentor (TM) provides youth under the age of 21 in any setting (i.e. home, community, etc.), structured, one-to-one, strength-based support services for the purpose of addressing daily living, social, and communication needs.
p>Qualifications: - A Bachelors' degree from an accredited school in social work, psychology, counseling, human development, nursing, sociology, criminal justice, public health, public policy, or gerontology, and at least two years of experience in counseling, casework or case management preferably with older adults and/or the disabled population.
