In addition to care coordination, the Health Coordinator provides direct client support within a structured residential environment, including; engaging clients in the milieu, observing and monitoring client changes in physical and in behavioral health status, reinforcing treatment plans and health education, assisting with access to healthcare services, and responding to urgent health concerns by notifying appropriate clinical staff or emergency services as needed. The Health Coordinator supports the intake process, coordinates internal and external medical and behavioral health appointments (including telehealth), assists with medication monitoring and documentation accuracy in the electronic medication administration record (eMAR/MAR), and gathers and communicates relevant health information to appropriate clinical staff.
