Responsibilities:

Responsibilities:
   <ul><li>Function as a clinician, educator and resource for patients within their specialty of care</li><li>Triage incoming patients ensuring compliance with established admission/acceptance criteria. Direct appointments and referrals as determined by the current criteria.</li><li>Develop, implement and evaluate the standards of practice and care for the patient population.</li><li>Demonstrates competence in the clinical skills necessary to carry out assigned job duties.</li><li>Responsible for tracking system for follow-up appointments related to office visits, laboratory tests and procedures to regularly follow patients monitoring disease status.</li><li>Answer questions and provide teaching to patients and family members. </li><li>Provide appropriate and timely documentation in the patient’s electronic health record using standardized workflows and processes. </li><li>Thoroughly understand testing (lab and procedures) values and relationship to patient status with accountability to take next steps, as appropriate to scope and licensure.</li><li>Responsible for the development and ongoing coordination of care for patients.</li><li>Act as a consultant both within the institution and to outside agencies or groups.</li></ul>   

Qualifications:
  <ul><li>Current RN licensure to practice as a registered nurse</li><li>CPR certification</li><li>Mandatory Reporter certification</li><li>Ability to direct others and to exercise judgement in problem situations</li><li>Creative thinking; analytical, problem-solving, and decision-making abilities; self-direction; theoretical implementation and evaluation</li><li>Strong interpersonal skills</li><li>Ability to work as a team member</li><li>Strong computer skills</li><li>Able to write, read, comprehend and speak fluent English</li><li>Computer data entry experience</li><li>Use of usual/customary equipment used to perform essential functions of the position.</li></ul>
