Key ResponsibilitiesClinical Leadership & Team Support:Lead, coach, and support a team of hospice RNs and CNAsServe as a clinical resource for symptom management, complex care planning, and end-of-life care decisionsFoster a positive team culture focused on compassion, accountability, collaboration, and excellenceParticipate in an on-call leadership rotation (approximately 5 days per month)Maintain a small patient caseload to stay connected to clinical careQuality & Regulatory Oversight:Ensure compliance with Medicare hospice Conditions of Participation and all state and federal regulationsLead quality assurance, infection control, and QAPI initiativesSupport survey readiness activities and plans of correction as neededConduct chart audits and clinical documentation review to ensure quality and complianceInterdisciplinary Collaboration:Lead bi-weekly Interdisciplinary Group (IDG) meetingsCollaborate with physicians, intake staff, social workers, chaplains, and leadership to support coordinated patient carePartner with leadership on clinical operations, staffing, and process improvement initiativesStaff Development & Operational Leadership:Support hiring, onboarding, training, and mentorship of clinical staffProvide ongoing coaching, competency validation, and performance managementAssist with operational oversight related to clinical supplies, DME, pharmacy coordination, and resource managementContribute to policy development and organizational growth initiatives QualificationsRequired:Active Minnesota Registered Nurse (RN) license in good standingMinimum 3 years of hospice experienceMinimum 2 years of hospice leadership, supervisory, or management experienceStrong knowledge of hospice regulations, interdisciplinary care, and end-of-life best practicesExcellent leadership, communication, organizational, and problem-solving skillsCurrent CPR certificationValid driver's license, reliable transportation, and auto insurancePreferred:Experience with QAPI, infection control, and regulatory surveysExperience leading teams in a growing hospice environmentEMR/documentation system experience About Suite Living Hospice CareAt Suite Living Hospice Care, we believe exceptional hospice care begins with strong clinical leadership and compassionate teams.
